Space Command 3.

You could accidentally jump to a system with no inhabited planets, have no settlers, run out of fuel, and have to restart/reload.

When that happened to me in Space Quest I could at least go jump off a cliff, to auto initiate the retry reload screen. (Or figure out what to do in the jungle, either or.)
