If I were to get AotI for my PC, I know I would have to pay for buying the game itself, but do I have to pay any monthly fee's or what?

There are 2 plans for starting subscription, but it's 9.99 a month.

Is there still extra mode on AotI, and if so, what chapter do I have to clear in story mode to get it?
