How do you upgrade them?

The uh... "meat" weapons are upgraded to medium and well-done versions through grinding.

Pretty much that yeah.
As far as I know, if you fail a grind they dont upgrade as well as they could.

If you get them to +10, they become "Medium". If you "fail" a grind they become "Well Done".

Well Done is B-Rank, Medium is A-Rank. They're all pretty weak weapons anyway though.

I thought you couldn't fail grinding in Portable?

You can't. The meat weapons are the only weapons that "fail" grinding, and even then they don't technically break, they just turn into burnt meat. Hence why I put quotes around "fail".
