Upgrade System

How exactly does the upgrade system work? I see the progress bar, but I've gotten multiple weapons to full and never had a failure.

Also, I see you can upgrade the weapons rank once it's FULL, but you need an item. What it is/Where do you get it?

You pay, it upgrades. Never fails. Goes up by a semi-random amount each grind (usually only +/- 1, some aren't random)

Need an extend code for a full weapon, get one by beating challenge mode. Can only get a limited amount per character supposedly.

Challenge mode is Online-only right? Like Bruce mission and that kinda stuff?

You can do it on multimode if you want, think you can even attempt it solo.

Ohhh no, limited for each character? I have a hard time picking out weapons to grind, let alone give a limited upgrade to.

Can anyone confirm that you can attempt the challenge mode solo? I've played a lot of multi mode today and found a lot of missions greyed out with the 2-4ppl notation on the bottom. However I don't know which option is for challenge mode, although I did go through all available options.
