You miss out on too many things if you dont start with the expansion from day 1. Sega is phazing out the original game itself...so its good to start on AoTI

yeah, i also agree with Keilyn that it's best you buy/start only the expansion.
The original game is kind of pointless when everything is in
the expansion. You'll only need the original if you want to know
the story of whatever happen to Ethan Waber and crews. And to me, the story
is kind of so-so, nothing to spectacular if you miss out on.

It depends a little on if you are talking about the 360 or PC/PS2.

On the 360, you have to buy the original game, then download the expansion. In other words, it is a proper expansion, not a separate game.

For PC/PS2, just get the second game - the only reason to get the original would be to play the offline story mode.

You can do all online story missions with just AotI (Episode 2 and Episode 3).
Episode 1 is only available on the original though, and only offline.
