To beat hard mode, do you have to beat all the acts and chapter? Or just just the last one?

All of them, in any order, as long as the 10th chapter is beaten last.

The enemies still level with you, right? As long as you're in the range of the levels shown?

Yes. They won't go any higher than the level cap shown in the mission description, nor will they go any lower than the minimum shown.
