Ranking Criteria

When completing a mission, you get a rank. (Yes, I'm captain obvious here) But what are the criteria for getting a S rank? I've figured out that the bottom is not dying, but I don't know what exactly the top one is and how you get it higher? I know it has something to do with kills/?mission codes? but not the criteria for raising these higher. Any advice on how to earn S ranks?

I believe its mission codes, however i'm not sure exactly how many you have to do to get it up.

First parameter is mostly based on how much you explore the maps and the rate you kill enemies around, so basically you cant simply rush through the maps ignoring all spawns around...the game measures those points based on the frequency you kill enemies. Second one is based on the times you die on field and return to the campship (0 deaths = full pts), but dying and being revived on field does not affect the parameter.
