Help With Login (Japanese)

I've downloaded the game and clicked on the icon. I was then instructed to click on the first box on the upper right which is the download patch, but when I clicked on it something popped up. Looks like it's asking for information but I can't enter anything into it.

As a side note, it can take a while to patch, so try to limit yourself when your're online.
