Second part asking for name need to be written in katakana. Mainly, katakana for how your name pronounces.

Is that what the error said or is that just what i need to do i traslated the page to enter information so i know what it says

Both? The fields 姓 (カナ) and 名(カナ) require the input to be in katakana so you... need to input the names in katakana.
