It's safe(ish). I suggest just the main patch and not the extras. I've had problems with those.
Start in Server 2 (Lots of English speaking people) or idk 10

Technically you can get banned for using it since it is a modification to the original game, but it is not super likely. I personally avoid using it (because I'm learning to read Japanese and have common sense >.>) but, like with any mod, it will have bugs here and there and can potentially harm the game / process client side.

1) Very safe, not at all possible to be banned for a client side change unless you go sending them an email or something stupid like that stating you're using it. This is no different than a skin mod, it's something that no one but you can see and cannot be picked up by the server, it is not a third party program.

2) Ship 2 if you're looking for the most english players.
