My heart goes out to him, I don't have a screen sadly, was a bit ragefaec'd to do that but here's how it went down:

Ability III 80%
Power III 90%
Spirita III 90%
Spirita Boost 100%

so I'm thinking, 'well, it'd suck if ability fell through but eh, it's kind of a longshot I even got it to III in the first place, might as well roll with it.'

I am now the proud owner of an Affix 1 cuil with Spirita boost.

...and nothing else.

It always did this to me. I could never succeed my affix if not ALL abilities have 100% success. I saw people in my guild getting 4 slots with one at 80% and one at 90% but never for me, as long as one had 90% it WOULD fail.
This made me hate the item lab and the rng of the game so much...
Its such a bad bad bad system: you cannot play with it and try affixes, you can only loose money and feel bad.
Leave this crap maso game.

How exactly do you get TechIV to 60% under normal circumstances? (+10 from boost week and +30 from booster, I presume).

Either way....ouch.

Thats why with high risk affixing, its really the best to go 100% on everything. Trading a 90% for an additional 5 t-atk really isnt worth the risk.

Well in PSU, if you'd happen to fail, you could no longer get to 10+ and you would've been reseted back to 0/9 instead of 0/10 and so on...

They did, but only before Ambition of the Illuminus. Afterward, they improved the system to that.

They did at first, but they patched it later because people didn't like it at all, to a system that was still pretty dumb, and could be fixed through (you guessed it) real money.

Then PSO2 is a upgrade from that but still pretty dumb.

It was a pretty dumb system through the game's whole life, but I was mollified by the fact that, for most weapons, grinding was something like a 20% improvement and not nearly double like it is now. But woe betide the fool that tried to grind an Agito Repca pre AotI.

You're missing the point its just no one likes seeing such good odds and losing out on 1 she had 4 affixes losing that Vol soul made everything else that went on it trash and that my dear friend is the biggest fail you can have happened to you.

This means she has to go through the whole process of re affixing everything and trust me you don't know how fucked up and bad that is which is why you fail to see the big picture.

No one likes having to re affix shit when something like that happens that's money you could have saved and in PSO2 you need every lick of meseta.

Since I'm one of the PSO GC players who directly jump to PSO2, so the matter regarding grinding (through Dudu) had surprised me. I had to pay to grind :p

You need to play more Super Robot Wars. It's much more common when you get hit with less than 10% chance.

More importantly, it's her choice to go through such decision like this one. That's what risk versus reward means.
