You have to complete a full run of the Tundra tower to get 1 Heartkey Catalyst.

No, that is the other way around. You can exchange Catalyst for passes. It's a 1:1 trade. But not a Pass for Catalyst.

No, but you have to clear all 60 floors.

If someone puts you up there, you won't get it.

Depends on if you have or have not completed stages before. At quest counter, select the XQ you want to do, and click on the option which says "View Personal Quest Status" (English Patch).
It should tell you up to which floor you've completed so far.

I.E. My Tundra XQ status says I'm on Stage 15, so I would have to start from Stage 16.

In fact, if you lead party, it shouldn't let you select higher stages. Easiest way to check.
