Yes to all 3.
I did Shiki, I'm up to Kuron Explore, and I just set to MB3 and changed blocks.
He doesn't do anything for me.

I haven't done the new area at all, matterboard probably doesn't matter, and are you sure you have unlocked the 2 x99 CO's from Shiki?

His very first CO is "clear free exploration Kuronia with me" at least on VH difficulty.
You need to have unlocked Kuronia exploration for it to appear, as well as having the necessary level I suppose.
My character is 75 in all classes and I'm the idiot who does every CO so if there's something else I dunno.

I haven't even done the first Kuronia mission and I have that CO.

He has more than one CO? He only has one CO for me the one where he wants to come with me to Kuronia.

How do you unlock the rest of the co's?

I got it unlocked and completed before the maintenance.
For some reason it unlocked when I changed blocks a couple times (well, more like went to my room a few times and picked emptier blocks when I loaded back in)

I don't see why it needed to do that though. Then again maybe it's one of the problems as to why they needed the emergency maintenance.

Only 1 for me, too.
Clear new exploration with him and his partner card as reward, nothing new appear after i cleared that one.

I think it was buggy, my character with everything unlocked didn't have it at first while my other character with almost not a single CO completed did. It randomly appeared a bit later.
