First question. You need some very expensive, very specialized equipment to fire charged particles at the hydrogen to cause it to combine and yadda yadda yadda... Basically you need millions to get it.

Second question. We do. The main problem with them is that they are not cost effective right now as they currently require more energy to run than they produce. The magnetic field required to hold in the plasma hydrogen is so increadibly powerful that it requires it's own powerplant to power it in many cases. Currently there are only a handful of countries that can actually build and use fusion reactors. Just look up fusion on the internet and you shall find a lot of information about it. Discovery Channel Networks and the such are another good source as they tend to do specials quite often on this type stuff.
