Body Color:Try not to pick any colors that are already taken, but if your Distinguish Feature is distinguishing enough, then you may have one already used.

Eye/Mouth Color: Both are the same color, except in rare instances like ToH's ninja bandana.>_> Only rule is not to have them the same color as the Body Color.

Weapon: Everybody kills people in this comic, leave this blank and you'll be fighting barehanded. Try to add variety to what already exists, I don't want to draw 30 swords.

Distinguishing Feature: What sets your character apart from the rest, ie Scrub's Eyes, Tact's/Sef's Cat Ears/Tail, Eanae's Blondeness/Chest...=O Be creative, but not impossible-to-draw creative.

Not really. That would be a Revive Ring, but those don't break. Instead, they can only work once a battle, and it's random chance, if you're asking how often.
