What exactly does the blue bar next to a weapon indicate? Do weapons break and/or need to be repaired?

Indicates PP, sort of like TP in the PSO. Except now you can execute special attacks with weapons that utilize PP

Oh and I've seen in some videos that it recharges - is this correct?

Yes ... You can find some cube that changes colors and pay meseta to recharge items, you can wait for it to recharge while you're in a combat area, or use an item to restore PP.
