What determines offline S Rank?

Time is a big deal offline.. If you go for empty treasure rooms you probably won't get S-rank.

Deaths often has nothing to do with rank offline, though sometimes the death of the weakest party member will cost you an S rank in story because he just so happens to be part of the S rank requirements (Like in the ruins, for instance).

I thought using items takes away rank offline too?
I was doing find and I had run like 3 S missions and was going for a fourth, did the exact same process like last 3 times, but this time accidentally used an item and got an A?

Im pretty sure offline S Rank deals with usually Time and Kills (Have to kill all enemies to get S Rank). But there are times like said before where deaths do count towards S Rank requirement and others its just about speed like alot of the story 'boss' fights.

what about on extra mode, i have tried and tried to get S rank and i never can. does anyone know if its the same for extra mode as story mode?

i dont know i have only gotten one s rank offline in extra mode and that was on evacuation i think it is next to impossible to get on the planets
