I joined with one of my regular mic buddies.. and not only could we not hear each other..we couldn't even see the other person's name flashing to indicate they were talking.

This is something you can normally see whether you even have a mic or not.

We couldn't do voice in our rooms, in the lobbies, or in missions. We were able to use an xb live private chat channel just fine, but neither one of us were able to use voice with anyone using the game channel and it was clearly a server side issue.

Shutting down consoles and restartgin did not help either. Anybody else experience this?

did you start directly into the game?
there is some "workaround" where you start the console with the tray open, log into live manually and then start the game. tried that?

For me, I went right to the system settings on my 360 console, and selected to not automatically boot up a game, in the drive. I find it better to always start the system with the system tabs. That way, I always make sure I'm fully signed onto Live before the game starts. You never know when your own Broadband service is temporarily slow, and your connection to Live is delayed until some point during the game-loading process.

Now given that, PSU's voice support is still not working 100%. It's much better than it used to be, but it still has a few bugs (particularly with wireless mics). Funny enough, all the other games, and the standard Xbox Chat works flawlessly with wireless mics, so I think Sega still has a little work to do.
