Google Authenticator wiped from phone and don't have Removal code

I am pretty much at a dead end at this point when a family member stupidly used the Smart Cleaning tool on my phone, it erased Google Authenticator on it and I am a bit distraught. I don't have the removal code either and I don't want to start over from scratch. What should I do?

Contact sega support and use coherent japanese when filling out their form.

That is a good suggestion. Problem is, I only know the birth date, email, and password. I can't recall ever setting up a security question either.
