Depends, I don't lose the boost as long the damage is under 700. The hits that make me lose hero boost would kill that dude instantly.

I calculate cost by how much I have to pay to purchase things I don't already have.
I personally don't consider fodder/Noble as costs for any particular affix because it's something I always have....
