This section of the guides will have most of what you want to know:

But it's a bit cluttered, and has all versions of the game mixed in. So these are the four guides you'll find particularly useful:
Forms 1-3 for Hunter:

Forms 1-3 for Ranger:
http://www.pso-world.com/sections.ph...cle&artid=1241
Forms 1-3 for Force:

All that info in one list

And if you're playing on GC, Xbox, or BB:
"Rare" Fourth Form Mag Guide:
http://www.pso-world.com/sections.ph...cle&artid=1031
The first three are just so you know the paths. There are four ways to get a bhirava. So long as you keep the stats in the proportions listed on the right class with the right ID, it should stay as a bhirava.
The fifth guide is what you'll probably want to know about, if you're on GC/XB/BB. To get those mags you need to make two certain stats equal to the other two stats (So DEF + POW = DEX + MIND, for instance) on an interval of 5 (100, 105, 110, etc up to 200). Since your pow and mind are both over 50 you can't do some of them, but the others you can still do. Note that the mag is required to be on the correct class AND the class needs to have the correct type of section ID.
If you don't want it to change into one of those fourth form mags and you're using a version where it can, just make sure def+pow is never equal to dex+mind. If you watch that and make sure the stats are right, you should be good!
