I've decided to make a guide on PA discs. For now I'm only going to do ones for melee weapons. Later on I'll try to do TECHNICs and Bullets.

Swords
Tornado Break: Can be bought at any weapons PA store. Note: Starting out as a hunter also gives you the skill
Spinning Break: AMF Recovery clear box
Gravity Break: Skill Master title reward. Recieved for lvling up 10 skills to lvl30
Knuckles
Bogga Danga: Can be bought at any weapons PA store.
Ikk Hikk: Explosive Maestro title reward. Reach Protranser lvl10
Bogga Zubba: Eco Protection clear box. Note: Starting out as a beast hunter; you start out with this as your first knuckle PA
Bogga Robado: Desert Goliath clear box
Spears
Dus Robado: Bought in any weapons PA store
Dus Daggas: Grove Of Fanatics clear box. Note: Hunters start out with this PA
Dus Majarra: Flying Berserker title reward. Reach Fighmaster lvl5
Double Sabers
Gravity Dance: Bought in any weapons PA store.
Spiral Dance: SEED Intel section title reward. Clear chapter 3 in story mode.
Tornado Dance: Whirlwind Slicer title reward. Reach Hunter lvl10
Absolute Dance: Eternal Traveler title reward. Clear chapter 8 in story mode.

Anga Dugrega: Bought in any weapons PA store
Anga Redda: Hill Of Spores clear box
Anga Jabroga: Mega Destroyer title reward. Reach Fighmaster lvl10
Twin Sabers
Rising Crush: Bought at any weapons PA store
Assault Crush: Lightning Beasts clear box
Splendor Crush: Claws and Bullets clear box
Cross Hurricane: Intel section title reward. Clear chapter 2 in story mode.
Twin Daggers
Hishou Jinren-zan: Bought at any weapons PA store
Renkai Buyou-zan: SEED Awakened clear box
Moubu Seiren-zan: Sleeping Warriors clear box
Renga Chujin-shou: Dark Satellite S clear box. Maybe Dark Satellite A clear box as well
Twin Claws
Bukuu Rensen-ga: Bought at any weapons PA store
Renzan Seidan-ga: Area drop in Hill Of Spores. Maybe in Dancing Birds as well
Rensan Senshou-ga: Moonlight Beast clear box
Chuei Jitotsushin: Light Speed Killer title reward. Reach Acromaster lvl10
Sabers
Rising Strike: Bought at any weapons PA store. Note: Hunters start out with this skill
Gravity Strike: Angry Soldier title reward. Reach Hunter lvl5
Spinning Strike: I'm not really too sure about this one. I've yet to see it myself. I've heard it can be gotten in Sleeping Warriors S clear box
Daggers
Shunbu Shouren-za: Bought from any weapons PA store
Buten Shuren-zan: Holy Ground clear box
Hikai Shuha-zan: Also not sure where this one can be found.
Claws
Shousen Totsuzan-ga: Bought at any weapons PA store
Senten Kanzan-ga: Moonlight Beast/Demons Above area drop (not 100% sure)
Bukuu Saien-zan: Rogue Family title reward. Clear chapter 5 in story mode
Whips
Visshi Grudda: Bought at any weapons PA store
Vivi Danga: Endrum Remnants clear box. Possbily s-ranked only.
Slicers
Chikki Kyoren-jin: Bought at any weapons PA store
Choutou Kantsu-jin: Hurricane Striker title reward. Reach Acromaster lvl5

A bit unrelated, but a nice tip I learnt from a friend was to never use underlines for text on forums and articles online because it makes it look like a link.

Great listings on all the PAs though, would have taken quite a long time just to list them all let alone where to get them.
