PSOBB: How to cancel PSOBB JP ?

Having not played anymore in the past months but still having a PSO account on the JP servers, I'd like to cancel it ; I remember back then I found some quite in-depth FaQ here with pics etc explaining how to register but can't find it anymore,

Any help would be greatly appreciated
