I wanna solve them all before I go get it. So what true, and what's not?

HL fee I know is true, but what is it?
US players can't play on JP servers?
Needs XBL to play offline?
Chiped XBOXs cannot access PSO servers?

And I'm sure there's more...

The game is not out yet, so there is NO ONE to tell you the exact truth.
However, having the Jp version, I can tell how that one is, since Us' should be really close if not the same.

HL : $8.95/month (Jp : Y600 ~$5 )

US and Jp players separated (official reason : security measure)

A gamertag is needed to play offline. If you don't know anyone with one and a mem card to bring it to your box, yes, you'll have to buy Live.
(Us version may have this requirement removed)

Chipped Xbox with chip turned ON cannot access Live.
Chip turned OFF Xbox can access.
