I finally got my hands on a broadband adapter but I can´t find the page to fill the form and get my license... so can someone help me?

In the instruction manual for the game you can find it. Or go to Sonicteam.com and choose your language then you should find your way to PSO and from there you should be able to purchase one. Or go online with your GC and choose website and from there you should be able to purchase it...
