Items are only translated if you use the Tweaker. If you are using the Tweaker, click the orb and check the box that says Translate items.

yeah i know,and it's checked and it still doesn't work

Sometimes the .dll that the Tweaker needs to download breaks. Nothing to do but just wait, or try restarting the Tweaker.
