does any1 know why this glitch keeps happening? (other games are fine)

i7 2600K
GTX 970 4GB
SSD 850 EVO 250GB
RAM 8GB...

everything in-game is maxed btw

You need to set pso2.exe to be able to use more than 2GB of RAM.
In the Tweaker, you can go to Other Tasks>Enable Large Address Aware.
If you don't use the Tweaker, then Shoterxx has created a nice little .bat file to do it available in this post.
