Combining items

How do you combine items? I've heard oh so much about it, I know it's how you get some good stuff such as Dark Flow, etc, but I was just wondering how.

And what's the 'Telepipe trick'? Is it the same thing as the Barta trick? I know I sound like a newb, but thanks in advance for answering my questions.
