GC-Item: Where do I find S-Rank weapons?

Well I was wondering where to find S-Rank weapons. I need to know if you can find them offline or online. Any and all help is appreciated as I haven't found one after a good half year of searching. (Which mode do you find them on? ie: Hard,Ultimate etc.) Oh and if you want to bother please answer this poll. -_- It is about which you prefer epsiode three or one + two.
