Combining items?

I'm proving my noob-ness again (and probably starting to get on some people's nerves) but I need to know...

How do you combine items? I know the Blue-black stone goes with the Monkey Bar (or something by a similar name) to make the Black King Bar...I really need to know how though @_@

Equip the weapon then go into your invintory and select the "use" option from the list when you look at the item. For example equip the MKB, then go into your inv. and pic use from the window on the blue black stone.
