When I see that term, I think of PSO. It happens with other games, but it's only happened to me on Final Fantasy 3 for the SNES once, and on PSO like... probably over 100 times.

well on the mario game i beat like 20 times.
but i had everything plus 99 lives

had that in metroid prime (once), need for speed most wanted (cube, three times, twice in chases), timesplitters 3 (cube, three times in story mode) and timesplitters 2 (cube, once, in a very eccentric custom map).
had it in pso (cube) maybe 20 times or more, had warpfreeze too. only lost (more or less easily) replaceable stuff.

my fsod-damage-prevention is:
-to never carry unequipped precious things.
-to equip the best weapon on entering a new level.
-to have expendables (shop rares, for example, which i bought in dozens)
-to never join a game from lobby 1

I think of the horrible beta drivers that I was forced into using for my video card due to some fixes that were in the betas but not in the full release drivers for about four months. Great job by that company, having the drivers kill an image editing program.

I didn't get FSOD too much on GC. Mostly what I got were the random disconnects due to craptacular wireless internet.
