Didn't work for me, even with 50+ runs of the first story mission. Couldn't get the True ending without all the other missions S-Ranked and correct answered. Though of course this doesn't disprove...
On both my characters, the same pattern occurred.
1) Beat story mode, bad rankings, random answers = bad ending
2) Repeat story mode, bad rankings, correct answers = good ending
3) Repeat story...
Then you got lucky, because the points system does not work like that.
Each time you start a Story mission, the points for that mission are reset to Zero. When you pick a correct answer, they go...
You must have S rank on every mission, check your scores in the visionphone.
There is no other criteria. Side missions are not needed.

Shouldn't be... I guess it could happen in a defective PSP. It's almost always down to corrupted installs, bad or unofficial memory cards, or being LittleBigPlanet.
No doubt CFW would cause issues...
The only thing that's ever frozen my PSP is LittleBigPlanet... and that's a hideous bug-ridden sh*tbiscuit of a game.
Almost certainly your PSP... though if you have the PSN version (as opposed to UMD) a re-install is definately worth a shot. It won't endanger your save data so there's nothing to lose by trying.
I always assumed that a second universe would appear if the first should become full. Never seen it happen though!

As far as I know, players are nearly always 'synced' to each other, so are bosses. Regular enemies are not, so they appear in different places to each player.
This happens in multimode...
I probably get D/C about 1 game in 10... and normally when that happens, it's the entire party that gets the boot, regardless of whether I'm the host. Kinda like when steam goes down.
What bugs me...
Is this Magashi plan an open mission or a Tactical or something? I can't find it and I'm levelling really slowly at the moment :(
In my experience Xlink Kai is pretty rubbish. Disconnects lots and is very laggy. Adhocparty for PS3 is much better, though still not great.

You have trouble with it? Has always worked fine for me, but then I don't seem to suffer any of the other lag-related issues people complain about aside from the slow-loading weapons and missed...

What's with those Perpa/Rainbow things that have broken text and some kind of % element resist. I vaguely recall hearing they don't effect resistance...
Having do re-do client quests, story mode etc. on alt. characters. At least Achievements stay, I guess.

I really like the stance... from what people have been saying I was expecting some slutty legs-apart ninja stance. Cool that it helps differentiate weapon classes.

Sounds almost 100% like corrupted memory card. Empty it, format and re-install everything. Though if it's already having trouble, card may be on the way to breaking.
Getting boosted helps lots.
My first character had Hunter:15 by level 60. I then boosted my friend from level 0 and he had Hunter:15 by level 40.
It hurts to see someone accomplish in 2 days...
