So i just need the stuff in green? That would be the 4th spawn or what?

Also, does all these things have to be the EXACT same things, and exactly is initial spawns for 1, 2, 3 and "x"

It's alarmingly simple. Try to follow along...

If, when you start the mission, the landscape before you looks like this:



Then you are playing map pattern "A" like it says in the chart.

If the first spawn you encounter is three Koltova ("Koltova x3") then you are playing spawn pattern A-1 like it says in the chart under the column labeled "Initial Spawn." This is NOT a rare spawn pattern, and if you are looking for a rare spawn you should just abandon the mission and try again.

If the first spawn is three Polty (Polty x3) then you are playing spawn pattern A-3 or A-X. It's too early to tell. Continue playing until block 3.

If the spawn in the cave is three Koltova (Koltova x3) then this is NOT a rare spawn map.

If the spawn in the cave is three Distova (Distova x3) then this IS a rare spawn map. Congratulations. Continue playing and eventually you'll encounter some Rappy.

If you are looking for a rare MAP then you were told several times that rare spawns and rare maps have no connection whatsoever, and you should just play.
