You should be aware that it looks like a ticket and you must go to the makeover shop to change heads (it's free). Also you can see all the cash shop items from the website.

The head comes from the normal, not golden, scratch card since the golden one doesn't give any cards. I suggest buying it from the auction house to gurantee that you've gotten the head. It should be cheap enough, if it isn't look back later. The cash shop items are always listed are bargain prices. (all the male cast parts cost me 30k altogether and that's because of the head) Heck my white tuxedo cost me 5k.

Is this a one-time thing since it's a ticket or do you unlock her head permanently?
It's a hairstyle, you can swap those as many times as you want.
