Does anyone know how these are generated? If I recall correctly in PSZ you got a friend code without even going online. So how does the game make certain that no 2 people have the same FC? This is something that's bugged me for, forever almost.

System/game generates fc, as well some aspect of the wifi used to initiate online activity with the game/system combo. Changing any of the three will result in a new fc.
What makes two people never have the same fc? The fact there are 12 digits, from 0000-0000-0000 to 9999-9999-9999, making for a total of 999,999,999,999+1 fc's possible... or a trillion, if you can't count all those 9's. I think if they only even used 1% of the numbers possible out of them all, they'd still have too many.
