The Phantasy Star Online Sympathy 2021 concert occurs in Tokyo, on April 24, 2021. The concert is being held as a part of the 20th anniversary celebration for the online game series.

The Sympathy 2021 concert will take place at the Bunkamura Orchard Hall in Shibuya, Tokyo, for both live and online audiences. Tickets to watch the online stream will be available for purchase at eplus from March 13, 2021, to May 2, 2021 — when the stream archive is set to expire. This concert will consist of Live and Orchestra segments with different instruments used for each segment.

The concert will be performed by the Tokyo Philharmonic Orchestra led by Masamichi Amano (天野正道) as conductor. Shouta Aoi (蒼井翔太), Mika Arisaka (有坂美香), Aimee Blackschleger (エイミー・ブラックシュレガー), Karen Flower (花れん), Florence McNair (フローレンス・マックネアー), Haruko Momoi (桃井はるこ), and Takenobu Mitsuyoshi (光吉猛修) will also perform at the concert.
