EDIT: Forgot to add, I also started a new character from scratch and only ran Episode 2 to see how simple it would be to unlock true ending in a single playthrough and to help those non-Japanese understanding folks asking about it:

S rank all story missions (unsure if actually required, but it's simple enough to do).
Don't kill Nagisa in 4-2 (and select all top options before the battle).
Requisite dialogue choices as listed on the Wiki:
Ch. 2 Act2, after boss: そうだ
Ch. 4 Act1, after boss: ナギサを信じている
Ch. 5 Act1, after boss: 決して諦めない (if this option occurs you are on track for True End)
For all other choices you can just select the top option and not care UNLESS you see ナギサ (Nagisa) somewhere in an alternative option, in which case pick that.
It is not necessary to ever take Nagisa on any Open Missions to farm 'friend' points, or clear any side stories.
