1. Drop wise I'm unsure as it was my understanding you could only get them from exchange.

2. Form "can" drop in Tokyo but otherwise only way you could was doing CF when it was around.

Certain LQs can drop them (I expect they have a higher rate than regular 13s, since I got three of them). Chaotic Roar of the Dragon is one of them.

Form Scythe can be found in Riding Quest, or as a Vegas Illusia drop (hence Nightmare Vegas Triggers being somewhat desirable for Form farming).

Urgent Maneuver 1. Always Urgent Maneuver 1. Doesn't matter what weapon it is, if it's Orbit, you take Urgent Maneuver 1.
