I haven't ordered anything too recently (nor do I do a lot of importing), but I've used PlayAsia a number of times in the past. To be fair, I usually ordered from them because they had a game on sale or because I shopped around and found they were the cheapest.
Actually, the last game I got from them wasn't even a Japanese release; it was the European release of the Tingle DS game. Still haven't played that yet...
I've gotten my games from PlayAsia. Haven't had any issues except once where I thought they never sent my J-3DS. Turns out it was stolen off of my front door. Worst Christmas ever. Just make sure you get tracking and insurance. Had to buy another out of my own pocket, but it went smooth that time around.
It's not the cheapest place, but it's reliable.

I often use Nippon-Yasan. They have a great selection and they always mark down the value of their packages so you avoid customs. And avoiding customs is a bit more tempting than the phonecards and mini wallscrolls that Amiami offers as pre-order bonus items. (´ヘ`; )
For secondhand stuff I use Rakuten, Mbok or Yahoo Auctions.
Almost always PlayAsia. But I bought Hatsune Miku Project Diva f off of ebay.
