Anyone able to translate this? Trying to Disconnect my friends SEN account from my Sega Account and was sent this email in reply

PlayStation Networkにサインインができない場合、『PSO2』
にてオンラインログインを行うことができなくなりますが、
『PSO2』に限らず、Sony Entertainment Networkアカウント
（以下、SEN アカウント）全般のお話にもなるかと存じます。
そのため、まずは下記ソニー様のお知らせをご覧いただき、
ソニー様へご相談いただけますようお願いいたします。

ソニー様へご相談いただいても、SEN アカウントが使用できな
い状況であれば、特別処置といたしましてお客様のSEGA IDと
SEN アカウントの連携を一度だけ解除いたします。

これにより、連携解除後にSEGA IDを、別のSENアカウントと
連携することで、『PSO2』へオンラインログインを行うことが
できるようになり、今まで使用していたプレイデータで引き続き
遊ぶことができるようになります。

なお、連携解除は一度のみとなりますため、別のSENアカウ
ントと連携した後に、再度パスワードリセットされたSENア
カウントのほうへ戻すことはできませんので何卒ご了承ください。

連携解除を希望される場合は、お手数をおかけしますが、
不具合報告フォームへ、解除してほしいSEGA IDでログイン
を行い、下記テンプレートの内容を投稿していただけますよう

I don't see anything about the account being unlocked in this email, so I'm not sure if it worked or not. I've tried translating it, but I still can't really make out what it wants me to do if anything. Anyone know what it might want me to do?
