Keyboard + Mouse Master race
Corsair K70 w/ Cherry mx Red switches and the Razer Naga Hex mouse. Love it!
Xbox 360 controller. I find keyboard + mouse extremely aggravating for any game besides top-down dungeon crawlers like Diablo or Torchlight.
Mostly because my reflexes are bad and my hands are perpetually shaking slightly, which throws precision aiming with the mouse off just enough to irritate.

Keyboard, because well I'm good at keyboard and mouse. I paid my dues with controllers, and will only use them for fighters and racers.
Dual Shock 4 controller with DS4 tool. Not a keyboard fan for these types of games because an analog stick gives more range of motion plus I don't like doing everything with one hand.
KB+M i can't into gamepad/controller button scheme.
