You can't. You would need to recreate the character. You do have a second, free character slot, though.

You can, but it costs 1500 AC and can only be done once every 30 days. You can access this from the second option once picking your character on the selection screen, and then the third option from there.

That's right, I forgot about them adding that in. *adds to compendium of knowledge* But, wow, $13 to change a character name; you can buy 3 new characters for that price.

EDIT: (Next bit is for my benefit; kinda thinking aloud type thing) Now I see why I didn't remember; very few places seem to actually mention the addition of being able to change Player ID/Character name. Top Google search was a thread from late July that brings it up, and I only found a single reference while digging through the bumped blog in the Livestream #18 post. Even digging though the JP wiki, I found a mention in the FAQ, but with no pricing, only a mention of being able to do it every 30 days.

No. You have to do it from the Character Select Screen, having enough AC.
(I changed my Player ID that was last week, which cost me 3000 AC. Ouch.)

I can't find any documentation that supports your statement. As of 7/17/2013, all accounts should have 2 available free slots; both the JP wiki and the game's official online manual support this (※3体以上のキャラクターを作成する場合はACが必要です。/ *AC is required when you create three or more characters.). If you have contradictory data, please post it.
