I always tell the doctor "Every time you reach inside of me, I keep a little bit of you." I find it breaks the tension and makes the immediate shift to awkward; but it's now awkward for them and not you.

Also, if someone looks like that when they're tugging the rubber glove on; just leave. Nothing good will come from that experience; unless it involves ripping out their throat and shitting down their neck tube.
