How do I add a new Item Find?

Found a Wals MK2 from a Skyly Run, in Ultimate Forest. This was a long time ago, I just remembered. It's not on the Drop Charts, so how do I add it?

(I'm sure of this: I got it from the boxes to the left of Central Dome before Sil Dragon, plus I would remember the 1st rare I got from Ult, wouldn't you?)
